The target must be one creature that is not in its natural form. The effect that turned the creature into the alternate form becomes permanent, and the creature loses the ability to change forms (if it had the ability.) Any effect that would change the creatures form from then on is negated, except as specified below.

If a spell that would allow the target to change forms is “Maximized” by the feat “Maximize Spell” or a similar ability, the target is allowed another saving throw. Success means the spell works, and the “Form Lock” spell is broken. Failure means that the spell fails, and the “Form Lock” remains. If the target fails the save, any attempts by the same caster to use the same spell to break the “Form Lock” fail until he gains another caster level.

“Dispel Magic” has no effect on “Form Lock”, but a targeted “Greater Dispel Magic” can be used, with a -5 caster level. Success means that the Form Lock is broken, and the target assumes normal form. Failure means that the spell fails, and the “Form Lock” remains. If the target fails the save, any attempts by the same caster to use “Greater Dispel Magic” to break the “Form Lock” fail until he gains another caster level.
